Fifteen patents, two programs

The phacoemulsification handpiece is the tool at the center of every cataract operation. Between 2021 and 2025 Dr. Wiley filed a family of designs reconsidering its tip — flexible and expandable geometries intended to change how the instrument behaves inside the eye.

A second program covers the pharmaceutical side of surgery: compositions for needle-free surgical sedation (2018–2020, co-invented with John Berdahl, MD and Mark Baum, assigned to Melt Pharmaceuticals) and, most recently, methods for extending the shelf-life of ophthalmic pharmaceutical compositions (2026, Harrow).

Technology, adopted early

Each of these implants arrived in the U.S. market with an adoption curve. Dr. Wiley was at the front of it — which meant Cleveland patients were among the first in the country to receive them.

  1. Light Adjustable Lens

    Among the first surgeons in the United States to implant the light-adjustable intraocular lens, allowing refractive outcomes to be tuned after surgery.

  2. iStent

    Early U.S. adopter of micro-invasive glaucoma surgery, combining stent implantation with cataract removal.

  3. Hydrus Microstent

    Early U.S. adopter of the Hydrus microstent for the surgical management of open-angle glaucoma.
